Out of School Hours Care (OSHC) Enrolment

Please complete this form for each child you are enrolling. Windermere OSHC must collect the information in this form to meet regulatory requirements.

The documents you will require to complete this enrolment are:

  1. Action plan - if your child has a diagnosed medical condition or allergy.

  2. Court order documents - if applicable.

  3. Imunisation documents.

  4. Medicare card.

  5. Additional needs supporting documents - if applicable (e.g specialist letters).

Permanent Care Details

Casual care means the child will have a flexible enrolment with no set dates.

Child Care Subsidy

In order for the service to provide Child Care Subsidy (CCS), the person applying for CCS must provide their Customer Reference Number (CRN) and the CRN of the child. These will be different for parent/ guardian and child.

  • Court orders, parenting orders or parenting plans concerning the powers, duties and responsibilities, or authorities of any person in relation to the child or access to the child?

  • Details of court orders relating to the child's residence or the child's contact with a parent or other person?

You must provide us with updated or amended court orders, parenting orders or parenting plans if they are changed.


4. Authorised contacts other than a parent/guardian

This section is for you to authorise other people to act on your behalf in certain circumstances.

  • You cannot include those who are already listed as parents / guardian 1 and 2.

  • You must provide two (2) authorised contacts.

  • You cannot authorise your Windermere Out of School Hours Care educator to be an authorised contact.

  • Only included people who are over 16 years of age.

5. Immunisation and health

Medicare details

Details of the child's doctor:

Please provide Windermere with the child’s immunisation History Statement (this can be downloaded from MyGov or Medicare).

Documents we cannot accept

  • The child's Health Record (blue book)

  • Health & Development Record (green book)

  • Overseas immunisation documents

  • Homeopatic immunisation documents

  • Statutory declarations

  • Conscientious objection and vaccination objection on non-medical grounds

  • Certificates from your doctor

*Windermere Out of School Hours Care do not site health records.

6. Medical conditions

Please upload the child's current Action Plan (within the last 12 months) for their condition. The child's doctor can provide this plan.

It is important that you ask for a copy of the Guidelines for the child's medical condition. This will help you understand how the educator will support the child.

We will also complete a Risk Management & Communication Plan with you before the child commences care.

7. Additional needs

Windermere is committed to providing an environment that values and respects the needs of all children to fully participate.

Example include a paediatrician, an early intervention service, therapists, Preschool Field Officers and an Inclusions Support Facilitator.

8. Filming and photography consent

A parent/ guardian's consent is required before images and recordings can be taken of the child.

12. Payment of fees

I/We authorise and request HubHello Pty Ltd ACN 160 176 018 to debit payments from my/our account, as specified below, at intervals and amounts as directed by Windermere Child and Family Services (“The Business”) as per the terms and conditions of my/our agreement with the Business and in accordance with the Direct Debit Request and the Hubhello DDR Service Agreement.

Your direct debit will commence Friday fortnight as per the invoice cycle. Debit amount = Balance Due
